PTA: Good afternoon, can I help you?

Customer: Yes, I’ve got a prescription here from my dermatologist. He’s diagnosed me with atopic dermatitis. I thought it was eczema.

PTA: That’s a common misunderstanding. Eczema is a broader term for inflammatory skin conditions. Atopic dermatitis is actually the most common type of eczema. Have you had it before?

Customer: Apparently, I had it as a baby and again in my early teens but not recently. The skin on my arms, my wrists and especially on my hands is red, scaly and unbearably itchy – look! I feel really self-conscious and I can’t shake hands with anyone. Is it contagious?

PTA: No, atopic dermatitis is not infectious at all and you’re far from alone. This skin condition affects around 10 to 20 percent of children and about 3 to 5 percent of adults.

Customer: Really? And what causes it?

PTA: The main factor is a genetic predisposition. People with this type of dermatitis often have a mutation in a certain skin barrier protein, which makes the skin more vulnerable. On top of that, environmental factors such as frequent hand-washing, contact with irritants, dry air, or emotional stress can trigger flare-ups.

Customer: Well, I’d say all those factors apply: I’ve just changed jobs and moved to a new town. How can I get back to normal?

PTA: The most important thing to start with, is to reduce itching and redness and that is best achieved with the cortisone derivate your dermatologist has selected for you.

Customer: I thought cortisone was dangerous.

PTA: That’s a common worry. Corticosteroids can cause significant side effects if taken internally or if strong preparations are used incorrectly over a long period. But your cream contains a mild cortisone derivative, designed for local use. When applied as prescribed, it’s safe and effective. The most impor-tant thing is to follow the instructions carefully and not to use more or for longer than recommended.

Customer: OK.

PTA: You will also need a good basic skincare programme to improve the disturbed balance of the skin and restore its barrier function.

Customer: And what would that include?

PTA: You should use a gentle soap-free cleanser and apply a soothing moisturizer. Good options are creams or lotions with urea, glycerol, or ceramides. They should be unscented. I can show you some products we have, and we could also prepare an individ-ual formulation if you prefer.

Customer: How long do you think it will take for the skin to get better?

PTA: I’m afraid you’re going to have to be patient. The first few weeks with a topical steroid should show some signs of improvement but if for some reason that doesn’t help, your dermatologist might consider prescribing a different medication.

Customer: Is there anything else I need to know?

PTA: Stay well clear of detergents and any perfumed cosmetic products; they would only make matters worse. Healthy eating and drinking are always a good idea. Stress reduction and rest can also make a big difference to your skin
